Clinical context

Visual acuity alone is insufficient when judging corneal disease or surgery. Topography and tomography, surface regularity, biomechanics, endothelial cell density, quality of vision and stability all matter. After refractive procedures, epithelial healing, dry eye, haze, ectasia and the need for enhancement should be assessed separately.

What was studied

This retrospective pilot series included 11 eyes with Fuchs endothelial dystrophy that underwent combined phacoemulsification, IOL implantation and Descemet stripping only (DSO) between August 2020 and January 2023. Selection was strict: central guttae spanning 4 to 5 mm, no corneal edema and healthy peripheral endothelium. IOL power was calculated with Barrett Universal II, and mild myopia was deliberately targeted because a hyperopic shift was anticipated.

How the evidence was produced

A cohort study describes how outcomes are associated with an exposure or treatment in an observed clinical population. This design is useful for routine practice, uncommon events and longer follow-up, but it remains vulnerable to confounding by indication, baseline risk differences and missing data. An association should not automatically be interpreted as a causal effect.

Key result Mean age was 68.3 ± 7.9 years and median follow-up was 16.5 months. All 11 corneas cleared without decompensation. Corrected visual acuity improved from 0.59 ± 0.48 to 0.18 ± 0.16 logMAR. The mean refractive target was −0.64 ± 0.14 D. Postoperative spherical equivalent minus the target averaged +0.33 ± 0.53 D, with a median of +0.18 D, indicating a modest hyperopic shift. The investigators found no significant change in astigmatism.

How to interpret the result

DSO retains the patient's peripheral endothelial population and avoids donor tissue, but central corneal healing may alter posterior shape and optical power. In this highly selected group, the planned myopic target partly offset the average hyperopic shift. The observation is useful for planning, but +0.33 D should not become a universal adjustment: variability was substantial, and the strategy already used a −0.64 D target derived from experience after DMEK.

The result should be understood as an estimate for the population studied, not a promise of an individual outcome. Statistical and clinical significance, absolute and relative effects, and prespecified and exploratory analyses should be distinguished. When several endpoints or subgroups are reported, the prespecified primary endpoint for which the study was designed usually provides the most reliable inference.

Clinical applicability

The study may refine technique selection and informed discussion, but its conclusions depend on baseline corneal shape, stromal thickness, equipment, nomogram and surgeon experience. Applicability requires comparison of these factors and attention to variability, uncommon complications and follow-up duration rather than mean values alone.

Limitations

This was a retrospective pilot study of only 11 eyes, without a control group or direct comparison of calculation formulas. Findings apply to eyes without edema, with localized central guttae and healthy peripheral endothelium; geometry and recovery may differ in more advanced disease. The source should be checked for keratometry methods, posterior corneal measurements, DSO zone size, use of ROCK inhibitors, IOL constants and the definition of stable final refraction.

Beyond the limitations acknowledged by the authors, confidence is affected by sample size, follow-up duration, missing data, choice of comparator and relevance of the outcomes to routine care. Uncommon complications may remain undetected even in a well-conducted study. External validity is reduced when participants, equipment or investigator expertise differ substantially from everyday settings.

What to verify in the source
