Clinical context

Myopia studies should distinguish refractive change from axial elongation. The measures are related but not interchangeable, and their clinical meaning depends on age, baseline myopia and follow-up duration. Studies of progression control should also assess tolerability, adherence, rebound after treatment stops and potential adverse effects.

What was studied

This single-center retrospective study reviewed 22,739 posterior chamber phakic ICL implantations performed from 2013 through 2024. Exchange was required in 110 eyes, or 0.48%. Cases were grouped as upsizing, same-size exchange or downsizing. The primary outcome was six-month change in vault; indications, age associations and endothelial cell density were also assessed.

How the evidence was produced

A cohort study describes how outcomes are associated with an exposure or treatment in an observed clinical population. This design is useful for routine practice, uncommon events and longer follow-up, but it remains vulnerable to confounding by indication, baseline risk differences and missing data. An association should not automatically be interpreted as a causal effect.

Key result Seventy eyes were upsized, 27 received the same size and 13 were downsized. After upsizing, mean vault rose from 311.6 ± 223.0 to 481.0 ± 278.6 μm, a gain of 169.4 μm, P<.001; the increase was smaller from age 35. Downsizing reduced vault from 1,284.2 ± 240.7 to 539.1 ± 176.1 μm, a fall of 745.1 μm, P<.001, and returned 12 of 13 eyes to the prespecified safe range. Same-size exchange reduced vault by 62.8 μm. Endothelial cell density declined slightly but significantly in every group.

How to interpret the result

Vault response depended on strategy: downsizing predictably reduced excessive clearance, whereas upsizing produced a more variable and age-dependent gain. Age may reflect changing posterior chamber and crystalline-lens anatomy, but it cannot replace direct measurements. Exchange planning should integrate repeated vault values, angle anatomy, pressure, endothelial status, toric rotation and symptoms rather than lens size alone.

The result should be understood as an estimate for the population studied, not a promise of an individual outcome. Statistical and clinical significance, absolute and relative effects, and prespecified and exploratory analyses should be distinguished. When several endpoints or subgroups are reported, the prespecified primary endpoint for which the study was designed usually provides the most reliable inference.

Clinical applicability

The findings apply most directly to patients who resemble participants in age, baseline refraction and rate of progression. Before selecting an intervention, check durability, the method used to measure axial length, completeness of follow-up and safety. A group average cannot predict the exact response of an individual child or adult.

Limitations

This was retrospective experience from one high-volume center in eyes already selected for reoperation. Only 13 eyes were downsized, so the apparent success rate is imprecise. Indications differed between groups, limiting direct comparison. A prespecified safe vault range does not guarantee freedom from late complications. The source should be checked for ICL models, exchange criteria, duration of endothelial follow-up and handling of two eyes from one patient.

Beyond the limitations acknowledged by the authors, confidence is affected by sample size, follow-up duration, missing data, choice of comparator and relevance of the outcomes to routine care. Uncommon complications may remain undetected even in a well-conducted study. External validity is reduced when participants, equipment or investigator expertise differ substantially from everyday settings.
