Clinical context

In glaucoma, intraocular pressure reduction is important but remains a surrogate outcome. Clinical value also depends on preservation of the visual field and optic nerve, safety, the need for additional medication or surgery, and treatment adherence. Target pressure is individualized, so the same numerical change can carry different meaning at different disease stages and rates of progression.

What was studied

Maintaining the intrascleral space may influence outflow after nonpenetrating deep sclerectomy. During combined cataract surgery, anterior lens capsule is available as autologous material. This prospective parallel trial enrolled 60 eyes of 60 adults with medically uncontrolled glaucoma and visually significant cataract. Participants received phacoemulsification plus deep sclerectomy with or without an anterior-capsule spacer. Minimum follow-up was 12 months, and the primary outcome was IOP change.

How the evidence was produced

Randomization reduces the influence of known and unknown confounders and makes comparisons between interventions more reliable. Readers should still examine sequence generation, allocation concealment, masking of outcome assessors, the prespecified primary endpoint, losses to follow-up and intention-to-treat analysis. These details determine how confidently an observed difference can be attributed to the intervention.

Key result Baseline IOP was similar: 25.14±9.62 versus 23.77±5.18 mmHg. Pressure fell significantly in both groups. With the spacer, IOP was lower at six months, 11.90±1.82 versus 13.13±2.20 mmHg, P=.022, and at 12 months, 12.00±2.20 versus 13.29±2.68, P=.042. Medication counts favored the spacer numerically but between-group differences were not significant. Under the stringent success definition of IOP ≤12 mmHg, survival was 47.5% versus 15.5%, log-rank P=.025.

How to interpret the result

The autologous material avoids a separate implant and was associated with about 1.2–1.3 mmHg lower mean pressure. That may matter when the individual target is low, but value depends on durability, safety and additional procedures. Medication evidence remained uncertain. The stringent success analysis supports a signal, although several success definitions require cautious interpretation.

The result should be understood as an estimate for the population studied, not a promise of an individual outcome. Statistical and clinical significance, absolute and relative effects, and prespecified and exploratory analyses should be distinguished. When several endpoints or subgroups are reported, the prespecified primary endpoint for which the study was designed usually provides the most reliable inference.

Clinical applicability

The findings can inform discussions about treatment sequence and burden. Before changing management, compare the study population with the patient in front of you, including glaucoma stage, baseline pressure, previous treatment, angle anatomy and rate of field loss. A pressure measurement does not replace structural and functional follow-up.

Limitations

This was a small trial of combined surgery, so the spacer cannot be separated from all other treatment components. Twelve months is short for late scarring. Assessor masking, postoperative standardization and rates of goniopuncture or revision require confirmation. Multiple success thresholds permit selective emphasis. Verify allocation concealment, glaucoma types and stages, primary analysis, attrition, IOP confidence intervals, complications, additional interventions, registration and funding in the full paper.

Beyond the limitations acknowledged by the authors, confidence is affected by sample size, follow-up duration, missing data, choice of comparator and relevance of the outcomes to routine care. Uncommon complications may remain undetected even in a well-conducted study. External validity is reduced when participants, equipment or investigator expertise differ substantially from everyday settings.
